HHC MG Earns Patient-Centered Care Recognition

We are excited to share that 12 additional Hartford HealthCare Medical Group primary care sites have earned Patient-Centered Medical Home (PCMH) recognition from the National Center Quality Assurance (NCQA), bringing the total number of recognized sites to 65. This achievement reflects our commitment to providing high-quality, patient-centered care.
Earning PCMH recognition is a rigorous process that drives improvements in how we deliver care. Each site undergoes a thorough evaluation to meet NCQA’s high standards for team-based care, access and quality improvement. Lauren Lappen, population health specialist II, leads the process, ensuring each site meets these benchmarks and excels in care coordination and health outcomes.
For our patients, PCMH recognition means easier access to care, better health outcomes and a more personalized healthcare experience. It prioritizes prevention, timely follow-ups and seamless communication through MyChart and other tools, helping patients stay connected to their care team and receive the right care at the right time.
